Overview

What is post-hospital discharge care?

Going home from the hospital is meant to feel like relief, but it often arrives with a thick folder of aftercare instructions, new medications, supply orders and a list of follow-up appointments that nobody has scheduled. The first days at home are when details are most easily lost.

I step in at discharge. I review the paperwork with you, make sure the medications and supplies are in place, visit your loved one at home, and take over the calls and scheduling that follow, so you can focus on recovery.

What changes when I step in
  • Review of discharge paperwork and aftercare instructions, in plain language
  • Medication reconciliation, covering what is new, changed and stopped
  • Ordering and picking up prescriptions and home medical supplies
  • A home visit to see how your loved one is managing
  • Scheduling every follow-up appointment and speaking with the doctors
  • Attending follow-up visits and bringing back the details
  • Weekly briefings as recovery progresses

Who It Helps

Who this helps

Post-discharge care helps anyone coming home after surgery, an acute illness or a long stay, and families who want a knowledgeable nurse in place before the first night at home. Research on nurse-led care after discharge has linked it with fewer hospital readmissions.

Common Questions

Questions about post-hospital discharge care.

Can you meet us before we leave the hospital?

Where possible, yes. I can review the discharge paperwork and aftercare instructions before you leave and be in place the day you get home.

How soon after discharge can care begin?

Most families begin within 5–7 days of their consultation, and urgent situations can often move faster. Call as early as you can.

What if the discharge instructions are confusing?

That is exactly what I am here for. I explain them in plain language and contact the hospital team or your doctors to clarify anything that is unclear.

Do you arrange the follow-up appointments?

Yes. I schedule the follow-ups, speak with the doctors and attend with you when you would like me there.
