Hospital Discharge Checklist: What to Do Before Going Home

Discharge day is often rushed. You are tired, your loved one is eager to go home and the instructions arrive in a stack of papers. A few minutes of preparation before you leave can prevent most of the problems that families run into in the first week.

This checklist is general guidance. Always follow the instructions from your loved one's own care team, and ask them to explain anything that is unclear.

Before you leave the hospital

  • Ask for the written discharge summary and instructions, and read them while someone from the team is still available to answer questions
  • Get a complete medication list that shows what is new, what has changed and what has been stopped
  • Confirm which prescriptions have been sent to the pharmacy and how you will pick them up
  • Ask about equipment and supplies, and who is arranging them
  • Ask which follow-up appointments are needed, and try to book them before you leave
  • Find out which symptoms mean you should call the doctor and which mean calling 911
  • Ask about diet, activity and any restrictions
  • Get a phone number to call with questions after you go home

Questions worth asking the discharge team

  • What was the main reason for this hospital stay, in plain words?
  • What should we expect over the next few days?
  • What should we watch for, and who do we call?
  • Which medications should we be careful with, and why?
  • Who is responsible for follow-up: the hospital, the primary doctor or a specialist?

The first 48 hours at home

Check that every prescription has been filled and that the home has what it needs: equipment, supplies, a safe path to the bathroom, a place to keep medications together. Set up a simple log for medications and symptoms, so that anyone who helps has the same information.

Compare the home medication supply with the discharge list. Medications are commonly changed during a hospital stay, and older bottles in the cabinet may no longer apply. If something does not match, call the pharmacist or the doctor's office rather than guessing.

The first week

Keep every follow-up appointment, and bring the medication list and discharge papers. Write down questions as they come up. Pay attention to changes in appetite, energy, confusion, breathing or pain, and report them early.

Many families find the first week more demanding than the hospital stay. It is a good time to ask for help.

How a nurse can help

I step in at discharge: reviewing the paperwork with you, reconciling the medications, ordering supplies, visiting at home and arranging the follow-ups. Research on nurse-led care after discharge has linked it with fewer hospital readmissions, and in my experience families simply feel steadier when someone knowledgeable is handling the details.

Let me take care of the rest.
